Taxpayer Preparation Documents For Filing
Please be certain ALL documents are available at the time you submit in-person, online or by fax.
PDF files are preferred. JPG or PNG is acceptable, but not preferred.
All Filing Statuses
State-issued ID - front and back
Copy of Social Security card
Income forms such as W-2, 1099, etc.
For Self-Employed Individuals
Proof of Income: 1099, W-2, and/or any cash or equivalent receipts for services.
Summary of Business Expenses, including but not limited to: Invoices, Receipts, Cancelled Checks, Annual Summaries, etc.
Other documents include:
Annual Accounting Ledger and/or Business Bank Statements if available.
Proof of any professional licensing.
For Individuals With Dependents
Qualifying Relative
Proof of residency showing relative lived at residence all year (bank or credit card statement, utility bill, etc).
Proof of income for less than $5050.00 in 2024.
Must provide more than half of your relatives total support each year.
Qualifying Child
Relationship: Child must be your son, daughter, step-child, eligible foster child, brother, sister, half-brother, half-sister, step-sister, step-brother, adopted child or an offspring of any of them.
Age: Your child must be under the age 19 or if a fulltime student under age 24. There is no age limit if your child is permanently and totally disabled.
Residency: Your child must live with you for more than half the year, but several exceptions apply.
Support: Your child may have a job but they cannot provide more than half of their own support.
Examples of Proof For Qualifying Children include:
Certified school letter and/or Physician statement
Form 1098-T
Copies of birth certificates and social security cards for each qualifying child
Copy of birth certificate and social security card for the parent(s)
Award letter for social services received
Agency placement letter for foster children
Proof of adoption
